Healthcare Literacy Toolkit

Our team developed a Healthcare Literacy Toolkit designed to help individuals better understand and navigate today’s complex healthcare system. Representing Puget Sound Kidney Centers, Tulalip Tribes, Providence Swedish, and Skagit Regional Health, our team focused on creating practical, easy-to-understand resources that empower people to make informed healthcare decisions with greater confidence. Through conversations with community members, we identified common barriers such as confusing medical terminology, uncertainty about where to seek care, and challenges managing medications and appointments.

The toolkit includes resources such as a “When to Go Where” guide for primary care, urgent care, and emergency room visits; plain-language explanations of common medical terms; a doctor visit preparation guide; a blood pressure tracker; and a “File of Life” emergency information form for the home. Designed with accessibility and simplicity in mind, the project aims to support older adults, caregivers, and community members in improving communication with healthcare providers, organizing important health information, and reducing confusion surrounding care. Our hope is that these tools will help strengthen health confidence, improve outcomes, and make healthcare easier to navigate for everyone.
